Public Notice: Urgent request for survey consent forms

The Ministry of Health requests parents and or guardians of students that attend the following learning institutions (schools) to return the signed consent forms for their child’s participation in the upcoming salt and iodine survey among school-age children (6-12 years), due to commence at the start of the new school term in April.

  •  St Andrew’s Roman Catholic
  • Telescope Primary
  • St George’s Anglican Junior and Senior
  • Alpha Junior

The ministry hopes that these forms can be returned to the respective schools, before the end of the current term.

The non-invasive survey will be carried out in the presence of a nurse and teacher, capturing urine samples, height and weight.

The ministry thanks you for your anticipated cooperation and looks forward to receiving the signed forms
